Case Management

Do you serve pregnant women?

Do you work with families that have children birth to 20-years-old with health issues?

Do your clients need to find resources to help them with their health-related (and other) issues?

If so, a case manager at the Family & Leadership Empowerment Network (FLEN) might be able to help.

Who can get a case manager?

Children (birth to age 20) and pregnant women who get Medicaid and:

·         Have health problems

·         Are at a high risk for getting health problems

What do case managers do?

A case manager will visit with clients and:

·         Find out what services are needed

·         Find services near where the client lives

·         Teach clients how to find and get other services

·         Make sure clients are getting the services they need

What kind of help can clients get?

A case manager can help:

·         Get medical and dental services

·         Get medical supplies or equipment

·         Work on school and/or education issues

·         Get basic needs assistance

·         Get other services
